AN AIR GAP WAS NOT OBSERVED BETWEEN THE END OF THE PVC DRAIN LINE FROM THE FOOD PREP SINK AND THE RIM OF THE FLOOR DRAIN BELOW. THE EXACT LOCATION AND IDENTIFICATION OF THE DRAIN LINE FROM THIS SINK WAS CONFIRMED BY THE OWNER AS THE PVC LINE ( APPROXIMATELY 5 - 7 FEET) RUNNING UNDERNEATH THE COLD BUFFET PREP COOLERS AND TERMINATING INSIDE OF A CIRCULIAR SHAPED FLOOR DRAIN. A DIRECT CONNECTION MAY NOT EXIST BETWEEN EQUIPMENT WHERE FOOD OR CLEAN EQUIPMENT IS KEPT. PROVIDE AN AIR GAP OF AT LEAST 1 INCH BETWEEN THE END OF EACH DRAIN LINE NOTED ABOVE AND THE RIM OF ITS RESPECTIVE FLOOR DRAIN. ITEM NUMBERS 1 AND 2 ARE REPEAT CRITICAL VIOLATIONS. PLEASE COMPLETE THE RISK CONTROL PLAN PROVIDED WITHIN 10 DAYS OF TODAY'S INSPECTION. A RE-VISIT INSPECTION WILL BE CONDCUTED WITHIN 10 DAYS OR LESS TO CHECK THE STATUS OF THIS REPAIR.
